The shrine is in Magreglio, near Lake Como in the Lombardy region of Italy.

The chapel is now part religious shrine and part cycling museum, with artifacts and photos from the sport. There is an eternal flame that burns there in memory of the cyclists who have died, and services each Christmas Eve and the Feast of All Souls commemorate them.

After World War II, Father Ermelindo Vigano, pastor at the shrine, proposed Ghisallo as the site of an Italian shrine for bicyclists. Pope Pius XII pronounced her patroness of cyclists on October 13, 1949.

In later times, the Madonna del Ghisallo hill was made part of the Giro di Lombardia bicycle race and has featured in the Giro d'Italia. The church sits atop a steep hill that climbs up from the shores of Lake Como, so it became a natural stopping point for cyclists.

Legend has it medieval Count Ghisallo was attacked by bandits when travelling. He ran to an image of the Virgin Mary at a shrine and was saved from the robbers. The apparition became known as La Madonna del Ghisallo, and she became a patroness of local travelers.
