Trump can certainly *try* to rule openly by force. It is becoming ever more apparent that when he sooner or later tries, we will see the kind of mass mobilization that can topple a consolidated dictatorship, let alone an almost completely unconsolidated one.

This I think is a really key point. No Kings has shown the ability to consistently get ~10m people out at a time when most folks still basically expect the 'system' to work (elections, etc). If Trump actually tried a coup, I expect we could double or triple those numbers.

The problem there (for inpatient less so for outpatient) is that the most dangerous time to be a patient is right after shift change, so the whole US medical profession has moved towards 12 hour shifts, and when you add in sick calls and crap it’s easy to get very long shifts.
