Caveat: some meds are designed with certain pharmacokinetic goals in mind. Aminoglycosides, for example generally work better with higher peak concentrations, so splitting the dose up is counterproductive despite achieving a smoother concentration

In Lupus nephritis patients, neutrophils have increased surface LAMP1, which is externalized after activation. Soluble LAMP1 is increased both in the blood and urine of patients, correlating with histological activity.

Yes you should be able to reduce medications after adding Spironolactone. I withdraw based on other adverse med effects (edema = CCB; K = thiazide/RAASi) I use this figure in my talks on Resistant HTN...(observational, but holds up imo)

Physiologically it never made sense to fluid restrict HF patients without hyponatremia. No difference in outcomes in 👇study, which was predictable. At the same time - from a study perspective - the actual difference in fluid intake between groups was <300cc/day.

It turns out, that under the right conditions, carbonyl groups form on MPO. Hydralazine actually ATTACHES to these carbonyl groups, probably modifying the MPO conformation and inducing a neo-antigen, which eventually causes the formation of anti-MPO IgM!
