Treatment Approach: 💊 Novel therapies for IgAN have shifted the paradigm to a bi-pronged approach. a) Generic responses to HTN/proteinuria b) Fundamental drivers of nephron loss by reducing the formation of aberrant IgA molecules & the maladaptive mechanisms that contribute to CKD.

The recent guidelines are evolving as a ray of hope in the evidence-based treatment of IgA Nephropathy, but the trials are conducted only in few countries and lack of data from the countries where the disease is more prevalent like India, questions the generalizability of these trials.

Special situations: IgA with AKI 🆘: AKI may occur during episodes of gross hematuria, usually improves after control of hematuria. Sometimes RPGN masquerades as AKI, so act early and have a low threshold for renal biopsy if there is uncertainty.
