In 2002 the U.S. military staged a war game against an Iraq/Iran-like adversary. Within 10 minutes the adversary took out 19 Navy ships, so the game was restarted. Even with constraints put on the opponents, the U.S. forces could not achieve their goals. Apparently nothing was learned from this.
