Histoplasma Antigenuria Prevalence in Patients With Advanced HIV Disease in CĂ´te dâIvoire: A Prospective Trial Ancillary Study
Despite tuberculosis being a well-known concern in patients with advanced human immunodeficiency virus (HIV), the STATIS trial, which focused on its management, highlighted significant mortality rates. Histoplasmosis, a fungal disease endemic in sub-Saharan Africa, presents with similar clinical manifestations as tuberculosis. Therefore, it may be prevalent and potentially responsible for deaths in patients with advanced HIV in this region. We conducted an ancillary study of the ANRS STATIS trial to provide the first prevalence estimates of histoplasmosis among individuals with advanced HIV in CĂ´te d'Ivoire.MethodsWe analyzed urine samples from patients previously enrolled in the STATIS trial in CĂ´te dâIvoire. These ambulatory patients with newly diagnosed HIV infection, CD4+ T-cell counts <100/ÂľL, and eligible for antiretroviral therapy (ART) were randomized to receive either systematic or test-guided tuberculosis treatment. We performed Histoplasma antigen enzyme immunoassay on their urine samples.ResultsThe prevalence of Histoplasma antigenuria was 68/280 (24.3%; 95% CI: 19.5%â29.8%), including 52/280 (18.6%; 95% CI: 14.3%â23.7%) symptomatic patients. Of 22 tuberculosis cases documented at inclusion, 8 (36.4%) also had Histoplasma antigenuria. In patients who died within the 48-week follow-up, the prevalence of Histoplasma antigenuria was 15/42 (35.7%% 95% CI: 22.0%â52.0%) compared with 22.3% (95% CI: 17.3%â28.2%) in those surviving. These survivors had a higher body mass index, CD4+ T-cell count, and hemoglobin and platelet count than those who died.ConclusionsThe prevalence of Histoplasma antigenuria was comparable to that of tuberculosis, and histoplasmosis was potentially responsible for preventable deaths. Prospective studies are needed to confirm these findings and promote screening strategies in sub-Saharan Africa.

Tickborne Neoehrlichia mikurensis in the Blood of Blood Donors, Norway, 2023
The tickborne bacterial pathogen Neoehrlichia mikurensis has been detected in <1% of blood donors in Sweden. N. mikurensis can give rise to asymptomatic persistent infections. Up to 25% of Ixodes ricinus ticks in southern Norway are infected with N. mikurensis. We investigated the incidence of N. mikurensis infection among blood donors in this region. We detected N. mikurensis in the blood of 45/499 (9%) blood donors by independent PCR methods; 69% of those were repeatedly positive 1â7 months after the first detection and tested negative after doxycycline treatment. We tested 8/19 adult recipients of potentially infected blood; none tested positive for N. mikurensis at the time of testing (191â301 days after transfusion). Our study identified a very high rate of infection with N. mikurensis in blood donors in Norway; whether infection can be transmitted by transfusion of blood products, however, remains unclear.

Prediction of antimicrobial resistance from MALDI-TOF mass spectra using machine learning: a validation study
ABSTRACTMatrix-assisted laser desorption-ionization-time of flight (MALDI-TOF) mass spectra can be used to predict antimicrobial resistance (AMR) using machine learning (ML). This study aimed to validate the performance of ML models for AMR prediction using own and publicly available MALDI-TOF data and to test how these models perform over time. Mass spectra of Escherichia coli (n = 7,897), Klebsiella pneumoniae (n = 2,444), and Staphylococcus aureus (n = 4,664) from routine diagnostics (Germany) and the DRIAMS-A database (Switzerland) were used. Six classification models were benchmarked for AMR prediction using cross-validation (regularized logistic regressions [LR], multilayer perceptrons [MLP], support vector machines [SVM], random forests [RF], gradient boosting machines [LGBM, XGB]). Performance was prospectively observed for 18 months after training. The performance of AMR prediction evaluated by the mean area under the receiver operating characteristic curve (AUROC) was comparable between the DRIAMS-A data set and own data. The best predictive performance (classifier, AUROC) on own data was achieved for oxacillin resistance in S. aureus (RF, 0.85), ciprofloxacin resistance in E. coli (XGB, 0.83), and piperacillin-tazobactam resistance in K. pneumoniae (XGB, 0.81). ML performance was poor if training and test data were unrelated in terms of location and time. Performance (change in AUROC) decreased within 18 months after training for S. aureus (oxacillin resistance, RF: â0.10), E. coli (ciprofloxacin, XGB: â0.19), and K. pneumoniae (piperacillin-tazobactam, XGB: â0.25). The performance of ML for the prediction of AMR based on MALDI-TOF data is good (AUROC ⼠0.8) but classifiers need to be trained on local data and retrained regularly to maintain the performance level.IMPORTANCEMALDI-TOF mass spectrometry can be used not only for bacterial species identification but also for the prediction of antimicrobial resistance (AMR) using machine learning (ML). Such an approach would provide antimicrobial susceptibility test results one day earlier than conventional routine diagnostics. This is essential for an early targeted treatment to reduce mortality of severe infections. We show that the performance of ML for the prediction of AMR based on MALDI-TOF data is good (AUROC ⼠0.8). However, the ML models need to be trained on local data and retrained regularly to maintain a good performance.
