In Super Mario 64 DS, giant characters normally cannot crawl. However, reading a sign pounded halfway into the ground forces the character into a crawling pose. When giant, this results in the character's giant posterior completely blocking the entire screen.

I think about Mega Man Legends drawing the mouths at different angles relative to the camera about once a month.

Here's a simple one that applies everywhere: Be OCD about naming things always, and spend the time to fix spelling mistakes in your code, scene object names, layers etc This WILL bite you in the ass constantly if you don't. It sounds simple, but it will save you countless hours of time.

I think my favorite "placeholder that slipped into the final game" story was the menu system in Symphony of the Night. Like, the entire menu system. They just got too used to it and simply forgot to gussy it up for the final version.

DOOM fully rendered in CSS. Every surface is a <div> that has a background image, with a clipping path with 3D transforms applied. Of course CSS does not have a movable camera, so we rotate and translate the scene around the user.
