1/n One of the things that I find valuable in internal medicine in general is pattern recognition. And nephrology is not the exception. AKIs come in different flavors, we can smell the etiology based on the presentation and the way the serum Cr (sCr) rises.

18/n so maybe the drug crystals cause injury as they travel down the nephron but they don’t end up absorbed into the renal parenchyma. As an example, atazanavir crystals do (PMID 32647771)

under 🔬. First, we were surprised by copious amount of colorful uric acid crystals. Serum uric acid was only 7.4.

11/n during literature search, I found a paper that claimed the 2-panels image below to be acyclovir crystals. Actually, those are NH4 biurate crystals (see example next to it). Remember: being skeptical is a healthy habit. Tons of published case reports are fake news and reflect poor peer-review

9/n Interestingly, several reports describe macroscopic visualization of crystalline sediment in the Foley bag that prompted

7/n more recently, we encountered another case of AKI with a precipitous rise in sCr in a pt treated with IV acyclovir (see graph). We were consulted about 36 hrs after the last dose of acyclovir had been given. The ask was to determine if acyclovir was the cause of AKI or not

5/n I have repeatedly emphasized that vancomycin is notorious for causing precipitous AKI. In the case of vancomycin however, there’s experimental evidence of vanco-mediated disruption of OATs that could drive some of the steep sCr rise. A past tweetorial here
