Radical idea: cancel all insurance. Start over. Hospitals and doctors will have to charge people what they can afford. Starting with Medicare, insurance has turned medical care into an unaffordable disaster of haves and have nots.

I love our medical system. If I miss an appointment, they charge me. If my provider calls in sick last minute and they miss an appointment, they reschedule and I pay either way.
