Yeah - you see the same basic dynamic in lots of different situations. Once the feedback loops to keep decision makers tethered to reality break down, then crisis is inevitable.

I've seen this a lot in the corporate world. Total loyalty to the leader is demanded and anyone that questions is deemed disloyal and PNG - even if the suggestion was made in loyalty good faith.
