PEM is the hallmark symptom of #ME and also occurs in patients with #LongCovid who will meet the diagnostic criteria for ME. It is not the same as what patients with MS experience, as this (otherwise good) article suggests. A 2-day Cardiopulmonary Exercise Test (CPET) can show the difference.

Could identifying the viral trigger of #ME/CFS inform future therapeutics? Heterogeneity is the name of the game when it comes to ME/CFS. Whilst ME/CFS is considered to be a symptom-based diagnosis, notably including post-exertional malaise ( #PEM), the core symptoms often mask a complex reality –

factors in which environmental factors, such as infections, may also be important (20) . The study confirms that a very large patient cohort is needed to uncover differences in genes.
