This week I realised what this dream had been about - the chance to inspire the Emergency Physicians of Germany, Austria and Switzerland (DACH) at
#DGINA2025
. The thing is, when you sit down to try to inspire people about what you do - the vocation, not just the work - something incredible happens.

Pediatric front of neck access (the surgical tracheotomy) An approach to the can't intubate can't oxygenate scenario in children

Pediatric front of neck access (the surgical tracheotomy)
Case It is the scenario of nightmares: a young child with an airway obstruction. They came in with stridor, and although you were initially thinking severe croup, the story doesn’t quite fit, there is no response to medical therapy, and this patient is incredibly sick. There is significant hypoxemia despite a nonrebreather. You can’t wait for backup, so you proceed rapidly to RSI.

💯 I remember one of my old boss teaching me “don’t make <insert management or government or funding> do their rationing. If you as clinician think patient needs an icu level care that is the end of the matter”. I reflect on this a lot actually

Elite athletes build rest into their plan. That’s accepted. That’s the norm. You need that to succeed. I’m not saying I’m an elite athlete - but I need rest to succeed also. It’s like always breathing in and never breathing out. Find rest today. It’s not recommended - it’s required.

super interesting! I boiled this down and simplifed/modified it to yield the summary below. I think we all do this to some extent but maybe should be more systematic about it. (and yes I'm publically admitting that I use femoral doppler instead of full VEXUS) (
