I’ve thought about this a lot recently. I met some amazing friends through Twitter, and built bits of my career that would never have happened without it. I sometimes feel guilty that I’m not available online in the same ways that others were when I needed them.

There is an institutional culture in many parts of the NHS of focussing wholly on money, and just not doing credible patient impact assessments when considering any cuts to service. Or worse, having them done and treating it as unimportant for many reasons nothing to do with patient health.

October is #BreastCancer awareness month. Our family is two months into our breast cancer journey. I’m incredibly grateful to my wife for checking regularly & spotting when something wasn’t right. The NHS is amazing & is here for all of us. Check yourself & get checked if it doesn’t feel right.

I’m really interested by the bits of this that are within our gift at department and programme level to action now, without big systems changes. Reducing the frequency of rotations, and publishing them early is an easy win. It just needs focus and the will to get it done.

This absolutely chimes with what I see in clinical practice. Gender incongruence is sufficiently common in young people with #FND that we should be screening for it, and the same with neurodivergence. Helping young people to understand their own brain is key to supporting all three.

I’m really excited by the pilot assessments coming to @rcpch.bsky.social training. It’s a positive move towards longitudinal assessment in the workplace. I think @orthopodreg.bsky.social will approve. A shame the #MedEd SoMe audience feels so much smaller now & it’s harder to get the message out.

Regardless of what you think about PAs/ AAs, if these recommendations are adopted in full then this is a sea change in how this group can work. Many people will see their jobs radially change or go entirely. We can be robust on standards of care while remaining kind.

I’m very curious but also unsure about this article. I see these associations in young people, but I worry that this article starts to fall into a seductive trap of suggesting causation (and therefore treatments) where there might not be.
