We've seen this before, using service utilization as a proxy. There are no outcome measures here. It rests on assumptions that don't make sense, and doesn't specify diagnoses, only that specialty services are for "severe disorders". This is diagnostically meaningless. It doesn't include symptoms.

The biggest issue is the outcome variable isn't "psychiatric morbidity." It's a proxy that is "contact with specialist psychiatric services." Kids in gender clinics are in constant clinical surveillance. Population controls are not. You are measuring detection, not disease. This is epidemiology 101.
