#PathSky 67M, long smoking hx, LN met. Keratin negative outside- “?sarcoma?” The hx suggests an undifferentiated pulmonary carcinoma with SMARCA4 loss. Our keratin was a bit positive and SMARCA4 is gone. These were originally thought to be thoracic sarcomas but have smoking related mutations.

Rare tubular patterned seminoma proven by OCT 4 with an unusual inter tubular pattern. On H&E, I thought a sex cord stromal tumour was a possibility. Never be ashamed of using IHC in the testis.

#PathSky Scalp mass, 33M. Just a nice example of soft tissue Rosai-Dorfman disease. I think this low power “pale->pink->blue” look is a good low power clue. In addition to S100 protein, Karen Rech’s group here at Mayo have shown OCT2 to be valuable in this dx. pubmed.ncbi.nlm.nih.gov/33177341/
