We know that black men have a higher chance of getting prostate cancer in their lifetime, but for most black men the cancer is curable when found. A good reason to consider a PSA check- prostate cancer is most easily cured when found early

67M, long smoking hx, LN met. Keratin negative outside- “?sarcoma?” The hx suggests an undifferentiated pulmonary carcinoma with SMARCA4 loss. Our keratin was a bit positive and SMARCA4 is gone. These were originally thought to be thoracic sarcomas but have smoking related mutations.

34M with a jet-black mass next to the seminal vesicle. Melanoma? A very occasional psammoma body was present, and expression of PRKAR1A was completely lost. Malignant melanotic nerve sheath tumor (previously “melanotic schwannoma”) in an unusual location.

Just learned about bluesky! For a first post, here is an image of ductal type prostate cancer. Diagnostic criteria can be controversial but common features are columnar cells and papillary structures. Typically considered grade 4.

Rare tubular patterned seminoma proven by OCT 4 with an unusual inter tubular pattern. On H&E, I thought a sex cord stromal tumour was a possibility. Never be ashamed of using IHC in the testis.

Scalp mass, 33M. Just a nice example of soft tissue Rosai-Dorfman disease. I think this low power “pale->pink->blue” look is a good low power clue. In addition to S100 protein, Karen Rech’s group here at Mayo have shown OCT2 to be valuable in this dx.
