Glad to have the opportunity to collaborate with Spanish colleagues on this. We conducted a pooled Bayesian post-hoc analysis of the BACSARM and SAFO trials evaluating combination therapy with fosfomycin for SAB. The door may not be closed on adjunctive fosfomycin! Justifies need for more RCTs.

A major new clinical trial, the Streptococcal Adaptive Platform (STRAP), has received $5 million in funding from the Australian Government’s Medical Research Future Fund (MRFF) to improve treatment for life-threatening streptococcal infections across all age groups. Interesting data Barista showing diversity in S. aureus isolates from sterile sites. Some evidence isolates become enriched for mutations over time. Wonder if really evolution over time or if samples from deeper sites would differ at baseline from those in blood?

Congratulations to Steve Tong, Josh Davis and the huge SNAP team (including Monash Health). Clear outcome that cefazolin is less nephrotoxic than flucloxacillin for MSSA, and pencillin is better for PSSA.

I enjoyed reading this review on Staph bacteremia by some giants in the field. Two highlights are the discussion of oral treatment options and the slight preference for cefazolin over ASPs. I rarely do cefazolin 2g q6 dosing, but considering using it more
