I am BEGGING major journals to stop platforming the current administration's politicization of the FDA, CDC, vaccines, etc This sort of evidence-lite, glitzy puff piece seems appropriate for a newspaper OpEd, not a medical journal. Platforming it in a major journal gives it the veneer of validity.

1/17 [Why] is furosemide susceptible to malabsorption from "gut edema"? When a patient with heart failure is hospitalized with congestion I often hear "let's use IV furosemide; they're probably not absorbing the PO." It's a comment unique to furosemide. But is it accurate?

Many putative mechanisms how RSR CaP can affect CaOx stone formation (Randall, heterogeneous nucleation etc), but also threats ( increase in urine calcium). This is why we need outcome data

SGLT2is likely inhibit citrate reabsorption in the PT. No need for alkalosis. Urine citrate and pH drove reduction in RSR CaP. Urine calcium went up in patients with calcium stones, no change in oxalate.

Interesting, important. Need UpH data. We can hope that CaP RSR is relevant to CaOx prevention; if Ucit went up why didn’t RSR for CaOx go down? Not sure how drug leads to ⬆️Ucit. Implies tubule seeing met alk but sHCO3 prolly didn’t change. For UA ⬆️cit implies ⬆️UpH

Watching Alien Resurrection (a movie set in year 2381) and the doctor uses a gonioscopy lens (a mirror designed to press against the eye to examine anterior chamber angle) as some sort of cross cover examination tool (a technique to evaluate eye muscles) and I have nobody to talk to about this.

Now that we have CysC as part of our BMP, I am trusting it less and less. Particularly in hospitalized patients who are inflamed at baseline. I just think this is a particularly instructive case because we could have just assumed that the creatinine was wrong because of her age and size.
