My only defense: audiobooks were much less of a thing when I started out, and when you read, your brain eventually sort of skips the dialogue tags. In audio, there's no skipping, you hear every single one. So it's far more annoying to just use "said" in audio. Anyway, now it's an addressed issue.

Serial orthopaedic surveys with systematic palpation of every bone and joint is standard of care in high energy pediatric trauma patients while inpatient, particularly those with polytrauma or distracting injuries, and imaging of anything identified therein.

We should stop looking at variability in expert judgment as some evil that needs to to be constrained with overwhelming standardization. Allowing variation and looking it in the eye is what validates the reproducible truth that physician autonomy improves patient outcomes.

I’ll be honest with you. After a night shift, I have often wanted a true self driving car. Preferably one that would feed me and then brush my teeth on the way home too… but… not these self driving cars. They are still very half baked by the looks of it. And not because they won’t brush my teeth.

It’s impossible to truly capture the added quality and saftey of a good sign-out with traditional metrics. You can’t just mandate “good sign out” because it takes all participants being engaged, interested and involved, and really is more of an ongoing team communication and feedback loop.

External calibration and incentives for clinical performance have failed us over and over, but we keep trying anyway. The underlying conditions inherent in making clinical judgements explain why this is true;

This is a great explanation of why we should all probably be asking trainees to at least tell us about the case before they scrub. I use planning, others use meetings, others use the scrub sink - but the idea is the same.
