As a GP, I initiate conversations about limited life expectancy when appropriate. They are less initiated by secondary care teams and patients sometimes disagree with me- ‘my consultant is trying to build me up’. An important topic for discussion particularly for secondary/primary care interface.

TL:DR - 1. Communication of poor prognosis from secondary to primary care doesn’t routinely occur, but GPs value it.
