30M with aggressive crescentic IgAN. M1E0S1T1C2. 4g proteinuria with Cr 160 umol/L but rising. Induction with IV CYC and pred. Now 1g protein but Cr, now 250 umol/L. Not sure what next :( maybe repeat biopsy?

My patients live at the margins of society. They donβt have much. Just getting to dialysis three times a week is a big ask. Taking from them to fund tax breaks for the rich is just cruel. I want to be outraged but I just feel like crying.

Prof Anders made a case for the (im)balance between workload (hyperfiltration) and nephron mass as the key interaction DM is a risk factor - there are no diabetic HF or diabetic cerebral vascular disease. Most cases are probably multifactorial HTN ckd probably doesnt exist and most are genetic

cut by more than half the level of kidney damage markers (urine albumin/creatinine ratio). In every single trial to date cutting UACR has led to kidney protection
