Needle tip placement does not need to be deep for either IS or ST blocks …the space is often only 0.5-1cm beneath the skin. No need to go hunting deep around the root origins!🤪

Agree absolutely however it doesn’t require “fascial plane block volume” 👌

I use an OOP approach avoiding going through SM and placing the needle tip in the plane between SM and C5 root juuust as the “traffic lights” start to converge.😊

We used to call superior trunk a "low" interscalene back in the day. Anatomically, all of the root to division blocks are interscalene because they are between the anterior and middle scale next muscles (insert at 1st rib). I treat interscalene more like a fascial plane block today.

The “traffic lights” are fine , you just don’t need to go advancing your needle too far 😊

I like both ISB and STB A true “root level” ISB is probably rarely done(I certainly don’t) Most are kinda a variation on a STB ..maybe just as the roots come together in a bundle as the subclavian appears..if I’m honest

I vary my sedation/music/chat depending upon need of patient. I also reassure them that this is under their control. They said I almost always employ Midazolam PRIOR to spinals/blocks to stop vasovagals/anxiety.

I agree that in limb surgery patients hate having a numb heavy limb…also true for Bupivacaine spinals for short procedures! More thought needs to be given to the operative block v the peripheral motor sparing postop analgesic block
