The way to get younger doctors back to the bedside is with data the physical exam finding outperforms other care. This is exactly what the ANDROMEDA shock 1 and 2 trial has done for capillary refill time. This is also the reason POCUS has brought younger docs back to the bedside!

Love a well made sequel! But 10 percent absolute mortality decrease might have been asking for too much? Also why bicarb vs. no bicarb and not bicarb vs placebo? Have to wonder if no bicarb group received dialysis early because everyone knew they weren’t getting bicarb.

Love this change in the new 2025 AHA HTN guidelines. Hypertensive urgency completely removed. Asymptomatic severe HTN as outpatient , DO NOT send to the emergency room, and more importantly if in the ED, NOT to give IV meds, nor admit. Much needed evidence based change.
