I am determined to deplete the area supply of bromfed. So many kiddos with the flu in the last few days. But I’ve been pretty hardcore on the no antibiotics position with the parents.

One of the small rural hospitals has decided to stop stocking liquid amoxicillin because it is “too expensive”. This is the second dumbest thing I have heard today after a parent said they don’t vaccinate because it causes autism and they trust RFK.

This years flu vaccine is an imperfect match for the dominant variant, subclade K. The shot gives some protection, just reduced effectiveness. But you can’t convince most people of that, the vast majority of patients I have seen are saying they either didn’t get the shot or won’t get it next year. 🙄

Every year the holiday period makes me question my decision to work in rural emergency medicine. So many people flooding the ER with cold and flu or stomach issues.

4 hours into the ED shift and I’ve already seen three dental complaints, two ortho, one assault, one burn, two URI, and two abdominal pain with V/D. Definitely the day after Thanksgiving here.

Made it to the Air Medical Transport Conference in Omaha. It will be interesting to see how my presentation on the rise of vaccine preventable diseases and managing these patients in transport will be received.

Made the trek to New Orleans to participate in the legislative general assembly of ENA. Once again advocating that membership and classes are unaffordable for nurses in developing nations.

Well it happened. Yesterday a patient insisted on being admitted for URI symptoms because he was convinced he had the covid variant that kills you he read about on Facebook. He wouldn’t listen to my explanations and left AMA to go to another hospital that “isn’t a liberal shithole.” I can’t even.

Can confirm that the vast majority of Covid patients I am seeing in the ER complain of sore throat, either “razors” or “ground glass” as the descriptor.

Over the last few days, the RN manager for the ED resigned and 1/3 of the staff ED nurses have also resigned. This is not good.

Always take a RN with you when it’s time to have the hard conversations with family of patients. When they need to, few in healthcare can provide such a compassionate presence. Although look closely at which ER nurse you pick, they can fool you.

This is a war without heroes. My googly eye reign of terror continues, radiology is the newest victim.

As someone who works at a hospital that cares for incarcerated patients, I can attest to the poor treatment many inmates receive behind bars. Many of them aren’t brought to the ER until their condition has progressed to serious or critical. It’s not humane. #MedSky

Over the last few months I’ve thought how my WW2 vet grandfather would feel about things today. He hated the nazis with every fiber of his being. He hated what they did and what he saw in Europe. He struggled with seeing so many of his peers dying in WW2. He would be mortified at the US today.

It was a fun meeting doing item writing. I feel like I learn a lot from my colleagues when we debate questions and the medicine behind it to determine if they will be good questions. Now a day or two off to rest before it’s back to the dumpster fire of US healthcare.

I get most of my aggression and mean-ness out when I got to a standardized medical exam item writer meeting.

One of the things I still love is fixing a nursemaid elbow on a kid. It’s so satisfying to diagnose, reduce it, and see the kid immediately improve and bouncing off the walls.
