Great article overall. I'm still of the thought these are the most important medications we've created since antibiotics. The depth and breadth of things these drugs help with is honestly incredible and almost unbelievable at times.

Here's my fun Friday morning question, do any my fellow PCPs/women's health providers use the lipid profile, specifically LDL and HDL to help "predict" onset of perimenopause? Obviously there are some other symptoms that usually occur but just curious if I'm the only nerdy one

Data from the Coral Reef Addon trial. Oral PCSK9I Enlicitide vs Zetia or Nexletol for deeper cholesterol reductions when added to a statin. Up to 64% reduction in LDL at day 56. Good LORD. With some patients over 80% reduction in LDL! Also 55% reduction in ApoB, really impressive results.

Question for my fellow GLP1 prescribers: Do you routinely prescribe or maintain patients on thiazide/loop diuretics if they're also on a GLP-1 med? (Excluding heart failure patients) I actively don't and will even change them to something else. I've seen too much pre-renal azotemia at this point.

The prior authorizations are bad enough with insurance. It's another level ENTIRELY when they have a drug as a preferred option THAT ISNT EVEN MADE ANYMORE. Byetta which was the first GLP1 drug ever, is the "preferred" GLP1 med for diabetes. Except it was discontinued 2 YEARS AGO!!

Massively unmet need. As of right now only 2 companies are studying any sort of incretin in type 1 diabetes. Eli Lilly with tirzepatide and Roche with CT868. The first Ph3 tirzepatide trial will wrap in Dec 2026 and the second in summer 2027. Could be a game changer for Type 1 diabetes
