The principles of support-centered housing have also been fundamental to the U.S. Department of Veterans Affairs, which has cut veteran homelessness by 50% over the past decade and functionally eliminated veteran homelessness in CT, DE, VA, and dozens of other cities and municipalities.

Support-centered housing has decades of robust evidence showing that this model is the most effective method to reduce homelessness. Support-centered housing reduced homelessness by 88%, improved housing stability by 41%, and provided greater health benefits compared to the housing first model.

Karen, NP, CCH: We approach the care of our clients on the assumption that everybody has trauma, and it's kind of the chicken or the egg: Did they have the trauma before homelessness, and that led to homelessness, or do they have the trauma because of homelessness? And usually the answer is yes.

Winnie, Program Manager, CCH: Homelessness is not just a housing issue, it's a public health crisis rooted in trauma, and this is where our integrated care comes in at the Coalition, and understanding that this is the point where people begin to heal--where our clients begin to heal.

Chronic illnesses are both more prevalent and more severe among people experiencing homelessness. Rates of conditions like heart disease, diabetes, and respiratory illnesses are 3 -6x higher compared to housed individuals.

53.9% of people experiencing homelessness report 4 or more ACEs compared to 3-5% of the general public. An ACE score of 4+ is linked to chronic health problems, mental illness, and substance use problems in adolescence and adulthood. Negatively impacts education, job opportunities, etc.

For four decades, CHH has maintained an integrated approach to housing and healthcare in response to the increasing crisis of homelessness and its devastating impact on public health.
