In Super Mario Galaxy, a planet can be seen in the distance in Good Egg Galaxy that should not exist. Attempting to fly to that planet makes it transform into a different one when Mario gets close. This is actually due to a completely wrong low-polygon model being assigned to it.

Ive seen people complaining about calling Deltarune Chapter 3's crystal boss a secret boss, and I get its not 'secret', but referring to it as a secret is a good way to not spoil players as to what it is.. Of course, it doesn't help much considering youtube exists and will spoil you anyway. >:(
