I have been researching oxygen therapy in ICU for many years. Here is something, that I have noticed. Apart from one small single centre trial (Oxygen-ICU), the point estimate effect on mortality for every trial in heterogeneous ICU patients has favoured liberal oxygen.

I think that could work just fine, especially given the kinetics of NfL compared w some of the other biomarkers, some of which peak early enough that they may not help after 72h.

NfL outperforms all other tests. If the story is bad and the clinical examination is bad, it seems reasonable to use the NfL in preference to MRI and EEG and stop if NfL is high. I wonder if we should just measure it in those who fail to wake (when they fail to wake). #CCR25

It is highly plausible that the MAP targets matter and that the optimal MAP target depends on the clinical scenario. This is an important trial but we need more trials in a range of populations to optimise MAP for individuals and populations. link.springer.com/article/10.1...
