I feel like almost no one calls either of these three by their code names in universe. That goes for enemies and allies alike. Are there other characters that have a code name yet people almost exclusively use their actual name?

Are Viltrumites planetary? No. It took 4 people working together to destroy a planet. It's the same in the show and in the comic. Viltrum is probably bigger than Earth but unless it's at least 4 times bigger I still wouldn't say they're planetary.
