How fast are you correcting hyponatremia? Scared of demyelination? Did you know that slower rate are associated with more death??

Big practice change coming? How fast do you correct hyponatremia?
When managing hyponatremia, the primary concern (at least as far as I have always been taught) is osmotic demyelination syndrome. We are warned not to correct the sodium too quickly, because the neurologic outcome can be awful. However, demyelination is very rare, and apparently there is a lot of data that suggests correcting sodium too slowly can increase mortality!

IV versus IO for cardiac arrest (PARAMEDIC3 and IVIO) No differences seen here. Might that be because vascular access is irrelevant if medications don't help dead people? #FOAMed

IV versus IO for cardiac arrest (PARAMEDIC3 and IVIO)
In the most recent BroomeDocs podcast, we talk about an attempt to use the IM route to give cardiac arrest victims epinephrine faster. (The podcast and Research Roundups blog post might actually come out next week.) I was skeptical about the outcomes, partly because the evidence that epinephrine helps at all in cardiac arrest is rather shaky. However, if you really want to give epinephrine quickly, it seems like the intraosseous route is the better way to do that during an arrest.
