(3/4) If a character is smiling while angry, it usually represents derangement or murderous intent. It can also be used to show confidence or cockiness, that they have a large ego. Usually a smile while angry is saved mainly for villains or 'firecracker' characters that are already usually angry.

(2/4) A character displaying anger doesn't mean that the audience also needs to also be angry. In fact, anger is often used for comedic purposes. If it's meant to be funny the character will often deform into a chibi or less realistic appearance.
