Candidate MRI biomarkers for CD79B status in primary CNS lymphoma: an exploratory radiogenomic analysis of the UCSF-PCNSL cohort - Journal of Neuro-Oncology
Purpose To test, under a pre-planned clinical framework, whether simple multiparametric MRI biomarkers show candidate associations with selected molecular alterations in primary central nervous system lymphoma (PCNSL), with a focus on non-invasive enrichment for CD79B status and the MCD-like phenotype of potential relevance to Bruton tyrosine kinase (BTK) inhibitor stratification. Methods The public UCSF-PCNSL dataset (146 patients; 63 with UCSF500 cancer gene panel) was analyzed. Four clinically motivated MRI biomarker-to-genotype pairings (CD79B, MCD-like, TP53, and CDKN2A/B) were defined in advance of any statistical testing. Effect sizes were reported as Cliff’s delta with 95% bootstrap confidence intervals, and one-sided p-values were adjusted for multiple testing across the four hypotheses at a false-positive threshold of 0.05. Reproducibility was examined by refitting each test after removing one scanner batch at a time (six batches total), and by benchmarking the habitat biomarkers against simple volumetric predictors using cross-validated AUROC. Results The principal hypothesis (H1) showed a higher enhancing-core (h1) fraction in CD79B-mutant patients (Cliff’s delta + 0.34, 95% CI + 0.04 to + 0.63; rank AUC 0.67; p = 0.015; direction preserved in 6/6 scanner batches). H3 was directionally consistent: higher FLAIR/T1c ratio in TP53-mutant patients (delta + 0.31; p = 0.049). MCD-like and CDKN2A/B hypotheses were not supported. After correction for multiple testing, no test reached the 0.05 threshold. Conclusion In this exploratory analysis, enhancing-core fraction on routine MRI emerged as the leading candidate signal for CD79B status in PCNSL, with FLAIR/T1c ratio as a directionally consistent secondary signal for TP53. After correction for multiple testing across the four pre-specified comparisons no test reached the 0.05 threshold; the findings should therefore be considered hypothesis-generating. External multicenter replication, ideally with full LymphGen classification, is required before these biomarkers can be considered for clinical use.

Background Systematic identification of potential deceased organ donors outside the intensive care unit (ICU) remains challenging. Patients receiving palliative sedation are rarely evaluated for donation, particularly in non-critical care settings. Objective To assess the operational feasibility of a semi-automated donor identification strategy triggered by midazolam prescription for end-of-life sedation outside the ICU. Methods We conducted a 12-month prospective, single-center feasibility study in a tertiary university hospital in Spain. A daily pharmacy-based screening system identified new midazolam prescriptions outside critical care areas. The transplant coordination team reviewed alerts, assessed medical suitability, and explored donation preferences when appropriate. When consent was obtained, intensive care to facilitate organ donation was initiated within a controlled donation after circulatory determination of death (cDCD) pathway. Results A total of 1,398 patients were screened (mean 3.8/day). Most alerts (94%) were excluded during rapid electronic review. Ninety-one patients underwent in-depth evaluation; 12 were medically suitable and 8 proceeded to cDCD, resulting in 19 transplanted organs. During the equivalent period in the preceding year, no referrals for organ donation originated from hospitalized palliative care services. Conclusions A semi-automated system based on midazolam prescription is operationally feasible and can be integrated into routine hospital practice to support systematic donor identification outside the ICU.

Textbook Outcome (TO) is a composite quality metric reflecting the ideal postoperative course through the simultaneous achievement of multiple desirable endpoints. Although widely explored in other surgical specialties, its application in endocrine surgery remains limited, and has not been specifically assessed in primary parathyroidectomy for sporadic primary hyperparathyroidism. A retrospective cohort study was conducted including 100 consecutive patients who underwent parathyroidectomy for sporadic primary hyperparathyroidism (PHPT) between January 2019 and February 2023 at a tertiary endocrine surgery unit. TO was defined by the absence of postoperative complications (wound events, symptomatic hypocalcemia, vocal cord dysfunction), hospital stay ≤ 24 h, histopathological confirmation of parathyroid tissue, absence of 90-day readmission or mortality, biochemical cure at six months, and absence of recurrence at one year. Descriptive and univariate analyses were performed. TO was achieved in 78% of patients. The most frequent reasons for failure were hospital stay longer than 24 h (n = 7) and lack of biochemical cure at six months (n = 7). Most failures (68.2%) were due to a single unmet criterion. In univariate analysis, absence of dyslipidemia (p = 0.016), single-gland resection (p = 0.022), and histopathological confirmation of a solitary adenoma (p = 0.026) were significantly associated with TO achievement. Application of TO to parathyroidectomy for sporadic PHPT proved feasible and informative. The analysis of failures identified prolonged hospitalization and lack of biochemical cure at six months as the most frequent reasons for non-achievement. TO may provide a comprehensive benchmark that complements traditional outcome measures and supports future quality improvement initiatives in endocrine surgery.

Bile leakage remains one of the most relevant complications after liver resection, contributing substantially to postoperative morbidity. Although minimally invasive liver surgery has reduced biliary complications compared with open approaches, robust multicenter data specifically evaluating bile leakage after robotic liver resection in expert centers remain limited. To assess bile leak incidence and severity following robotic liver resection in expert centers. This national, multicenter observational database included all consecutive adult patients undergoing robotic liver resection in three high-volume reference centers. Patients requiring biliary diversion or living donor procedures were excluded. Perioperative outcomes were prospectively collected, focusing on postoperative bile leakage, defined and graded according to the International Study Group of Liver Surgery (ISGLS) criteria. Secondary outcomes included overall morbidity, conversion rate, mortality, and length of hospital stay. A total of 492 patients were included. Malignant disease was the primary indication in 80% of cases, and 62% of resections were anatomical. Conversion to open or laparoscopic surgery occurred in 2.6%. Postoperative bile leakage occurred in 9 patients (1.8%). According to ISGLS classification, 6 patients (67%) developed Grade A bile leaks and 3 patients (33%) Grade B leaks; no Grade C bile leaks were observed. All biliary complications were managed conservatively or with minimally invasive interventions, without the need for surgical reintervention. Overall postoperative morbidity within 90 days was 29.3%, with a 90-day mortality of 1.6%. Median length of stay was 3 days (IQR 3–5). In this national multicenter cohort from expert centers, robotic liver resection was associated with a very low incidence of bile leakage and an absence of severe (Grade C) biliary complications. These findings support the safety of robotic liver surgery and highlight the key role of high-volume centralization in keeping biliary outcomes.

Nursing Students’ Experiences of Learning Evidence-Based Practice Through a Flipped Classroom: A Qualitative Study
Background: Evidence-based practice (EBP) is a cornerstone of high-quality and safe nursing care. However, undergraduate nursing students often experience cognitive, methodological, and contextual barriers to learning and applying EBP. Active teaching strategies, such as the flipped classroom, may support the development of EBP competencies, yet qualitative evidence exploring students’ learning experiences remains limited. Objectives: To explore nursing students’ perceptions and experiences of learning evidence-based practice through a flipped classroom model. Methods: A qualitative descriptive study was conducted at the Faculty of Nursing of the University of Murcia (Spain). Purposeful maximum variation sampling was used to recruit undergraduate nursing students from the second and fourth academic years who had completed an EBP course delivered using a flipped classroom approach supported by an online learning platform. Twenty semi-structured interviews were conducted via videoconference. Data were transcribed verbatim and analyzed using reflexive thematic analysis with independent coding by two researchers and consensus procedures. Ethical approval and confidentiality were ensured. Results: Three main themes were identified: (1) transformation of the meaning of EBP learning and professional role, (2) cognitive and metacognitive processes in EBP learning, and (3) the learning experience as a catalyst for deep learning. Students described a shift from initial fear and perceived difficulty toward recognizing the practical value of EBP, accompanied by increased critical thinking, autonomous learning, and a growing evidence-informed professional identity. The flipped classroom model facilitated engagement and understanding, while the transfer of learning to clinical practice was influenced by contextual facilitators and barriers. Conclusions: Learning EBP through a flipped classroom was experienced as a transformative process that fostered critical thinking, self-regulated learning, and the construction of an evidence-oriented professional identity among nursing students. Strengthening information literacy skills and improving alignment between academic and clinical environments may enhance the sustainable application of EBP in clinical practice.

Iron deficiency (ID) is the most prevalent nutritional disorder worldwide, affecting diverse populations including children, women of reproductive age, older adults and patients with chronic conditions. Oral iron supplementation remains the cornerstone of treatment, together with management of the underlying causes. However, conventional ferrous and ferric salts are often associated with gastrointestinal side effects, poor adherence and limited efficacy, especially in inflammatory settings due to hepcidin-mediated absorption blockade. This review summarizes iron absorption physiology, limitations of traditional oral therapies, and the potential benefits of iron protein succinylate (IPS), a ferric complex bound to succinylated casein. IPS provides pH-dependent release and contains succinic acid, which may enhance absorption while reducing gastrointestinal adverse events. Clinical studies indicate that IPS achieves hematologic outcomes comparable or superior to standard oral salts, fewer gastrointestinal effects and better tolerability. These properties make IPS suitable for patients who do not tolerate or respond adequately to conventional therapy. Special attention is given to chronic inflammation, pregnancy, cancer, or gastrointestinal disorders, where oral iron often fails due to impaired absorption or poorer tolerance. Practical recommendations are included to optimize supplementation through dosing strategies and tailored approaches. Overall, IPS offers an effective, better-tolerated alternative to conventional oral iron therapy.

Background/Objectives: DDX3X syndrome (MIM#300958) is a neurodevelopmental disorder associated with intellectual disability, language impairment, and a characteristic neurobehavioral phenotype that predominantly affects females. Although dysmorphic features have been reported, a consistent facial phenotype and clear genotype–phenotype correlations have not been established. Methods: We conducted an observational, ambispective, descriptive study including patients aged 0–18 years with a molecular diagnosis of DDX3X. Clinical, standardized facial images, neurobehavioral, neuroimaging, and molecular data were collected. Automated facial analysis was performed using Face2Gene after algorithm training. Results: Of 11 identified patients, 9 were included (8 females); 8 variants were de novo and 4 novel. Two variants of uncertain significance underwent in silico analysis. Frequent facial features included thin upper lip (9/9), tapered chin (8/9), long uniform eyebrows (8/9), short neck (8/9), and long face (6/9). After training, Face2Gene identified DDX3X syndrome in 92% of cases within the top 5 suggestions, supporting its utility as a diagnostic aid. All females had intellectual disability and language disorder; 66% presented sleep disturbances and aggressive behavior. Neuroimaging revealed ventricular dilatation (5/9) and corpus callosum hypoplasia (3/9). Loss-of-function variants were associated with greater clinical severity. Conclusions: This series suggests a recognizable facial phenotype of DDX3X syndrome and supports a possible genotype–phenotype correlation. Further studies are needed to confirm these findings.

Background The clinical presentation of melanoma of unknown primary (MUP) and its treatment patterns remain incompletely characterized. Methods GEM1801 is a prospective, observational study in Spain enrolling patients with advanced melanoma. This analysis describes the clinical features and outcomes of patients with MUP. Results From August 2018 to December 2022, 893 evaluable patients were enrolled, of whom 133 (14.9%) had MUP. Median age was 63 years (range 24–93); 81 (60.9%) were men; 117 (88.0%) had distant metastases; lactate dehydrogenase (LDH) was elevated in 50 (37.6%); and 79 (54.9%) had BRAF-mutated tumors. Compared with patients with a known primary, immunotherapy was prescribed less frequently (adjuvant: 15.5% vs 27.2%, p = 0.007; first-line metastatic: 53.6% vs 64.0%, p = 0.039). Median progression-free survival (PFS) was 18.9 months (95% CI: 4.4-NR), 13.3 months (95% CI: 7.1–27.5), and 6.7 months (95% CI: 3.5-NR), and median overall survival (OS) was 24.9 months (95% CI: 24.1-NR), 13.9 months (95% CI: 8.3-NR), and 17.9 months (95% CI: 9.6-NR) in patients with BRAF-mutated tumors treated with immunotherapy, BRAF-mutated tumors treated with targeted therapy, and BRAF-wild-type tumors treated with immunotherapy, respectively. Conclusions In Spain, MUP is characterized by multi-organ involvement and frequent brain metastases and was associated with worse outcomes than the overall melanoma cohort. A diagnosis of MUP appears to influence treatment allocation, particularly regarding immunotherapy. When stratified by BRAF status and treatment type, no survival differences were observed between MUP and melanoma of known primary, except for OS among patients with MUP and BRAF-mutated tumors treated with immunotherapy. Purpose To evaluate the association between latency duration and perinatal outcomes in pregnancies complicated by preterm prelabor rupture of membranes (PPROM) before 34 weeks of gestation, and to assess the role of gestational age at PPROM and amniotic fluid volume. Methods This retrospective cohort study included pregnancies complicated by PPROM before 34 weeks of gestation managed expectantly at a tertiary care center. Latency duration was defined as the interval from membrane rupture to delivery. Composite neonatal and pregnancy adverse outcomes were analyzed using latency-based time-to-event methods. Cox proportional hazards models were used to assess the independent associations of gestational age at PPROM and amniotic fluid pocket with adverse pregnancy and neonatal outcomes. Results A total of 278 pregnancies were included in the final analysis. Latency duration was strongly associated with gestational age at PPROM, with progressively shorter latency observed at more advanced gestational ages. Larger residual amniotic fluid pockets were independently associated with prolonged latency. In latency-based Cox models, gestational age at PPROM was significantly associated with both pregnancy-adverse and neonatal composite outcomes. In contrast, latency duration itself and residual amniotic fluid volume were not independently associated with adverse neonatal outcomes after accounting for gestational age variables. Conclusions In pregnancies complicated by PPROM before 34 weeks of gestation, gestational age at membrane rupture is the principal determinant of latency duration and perinatal outcomes. Latency should be interpreted as a gestational-age-dependent phenomenon rather than an independent predictor of neonatal risk, supporting individualized expectant management that prioritizes gestational age advancement while balancing maternal risk.
