YES Sindaes is canonically a trans man. WoW has a lot of options for changing one's body etc so probably some magical stuff went on when he discovered this Twitcher is NB! He dresses in whatever he wants, doesn't think of himself as a "man" ever and is just very comfy in his body and expression...
